Home and Community Based Support
Community Living Care’s Home-Based Waiver Program offers services to people with Intellectual Disabilities, within the person’s home and community. Direct Support Professionals (DSPs) work with people on multi-level tasks that are designed to fit the person’s individual need and schedule.
The people we serve are our first priority and the quality of service that we provide is our greatest responsibility. It is the intent of CLC to support a person throughout their life. As a person ages and their needs change, our services adapt to meet their current need. We look forward to having the opportunity to serve you and /or your loved ones in our program. Each person and their family choose their DSP and work together to create the best schedule for the need of the person.
Independence Is Happiness
We offer multiple services for adults and children through waiver and base funding, which include:
IN-HOME AND COMMUNITY SUPPORT
Permits the person to set and work on goals related to increasing independence within their home and/or community of their choosing. DSPs provide the support necessary to assist the person in achieving their goals.
COMPANION
Enables the person to participate in activities of their choice. DSPs will assist in creating and/or scheduling activities in or out of the home, based on the person’s need and desires.
RESPITE
Provides a needed break to the primary caregiver. DSPs assist the person with provision of care needed in the absence of the primary caregiver. This service can occur in or outside of the family home.
FAMILY AIDE
Provides a needed break to the primary caregiver of consumers under the age of 18. DSPs assist the person with provision of care needed in the absence of the primary caregiver. This service is only available through a county contract.
