Our in-Home support service workers are as unique as the needs of the people they serve.

To create a list of duties would be endless. Their role is dependent upon the needs of you and your loved one.

Our workers pride themselves on:

~Establishing a trusting relationship with the individual/representative and maintaining regular client contact.

~ Assisting in strengthening interactions and improving skills by modeling appropriate actions during all visits.

~Providing support to the individual to participate in new learning opportunities, aid in maintaining their current daily living skills, and promotion of healthy social interactions.

About Our Services

Home and Community Habilitation

These services can be provided in home and/or community setting to assist the consumer in retaining, acquiring and improving self help, socialization and/or adaptive skills. Services include but are not limited to, habilitation, respite, transportation, accessibility adaptations, *basic housekeeping, *minor repairs, *Home management, and *laundry assistance.

*These services are not available under PFDS or Consolidated Waiver

History of the In-Home Program

The In-Home Program began as Home Based Waiver in 1995 to provide respite and companion care to individuals with development disabilities. Our goal is to provide the highest quality of in home support utilizing information provided by the family to create custom care solutions. People we serve our are first priority and the quality of service that we provide is our greatest responsibility. We feel that communication is the key to a successful program and encourage you to contact us with any questions or concerns you may have.
